				<div class="et_pb_text_inner"><p style="text-align: justify;">We are running a trial using a surgical device that is currently under investigation so is not available for human use outside a clinical trial. The device has been used in over 85 patients worldwide and appears as safe as other currently used surgical methods. Eligible patients, who have undergone medical screening will be offered the procedure as part of a clinical trial. While we cannot foresee what results or outcomes may occur for people in the trial, these types of surgeries can allow patients to lose around 15% of their starting weight by a year after surgery, and an improvement in weight, or Obesity related conditions such as Type II diabetes.</p>

<h4 style="text-align: justify;">What does the surgery involve?</h4>
<p style="text-align: justify;">It’s a combined endoscopy and laparoscopy procedure which involves creation of a bypass between the duodenum and the distal small bowel, otherwise known as a duodeno-ileostomy. This leads to a change in hormones associated with eating as well as absorption of calories from food. Both of these together helps cause weight loss however they can also lead to diarrhoea and nutrient deficiencies if not combined with dietary change and vitamin/mineral supplements.</p></div>

<p><span style="font-weight: 400;">Single Anastomosis Gastric Bypass/Mini Gastric Bypass  (SAGB/MGB), also known as the Omega Loop Gastric Bypass (OAGB), is a more recent innovation in bariatric surgery. Unlike the RYGB procedure, it involves just one connection between the stomach pouch which is always long and narrow, and the small intestine. This approach leads to weight loss using the same mechanisms as the RYGB while simplifying the procedure. Some surgeons lengthen the small bowel bypass &#8211; this may possibly lead to more weight loss but will also increase the chances of persistent diarrhoea and nutrient deficiencies.</span></p></div>

This will tell you a lot about their experience and help to determine the chances of a positive outcome. <p><span style="font-weight: 400;">In reality, the costs for an uninsured patient at our clinic, or most clinics, can be broken down into two main areas; hospital costs and surgeon/clinic costs (including surgeon fees etc). Costs for both will vary depending on the individual patient and/or surgeon, for example, care required (such as ICU vs regular bed, length of hospital stay), or the complexity of the procedure (how long surgery will take). </span></p>
<p><span style="font-weight: 400;">If you were insured, it would cover your hospital costs and (depending on the provider/policy) other related expenses. But without insurance, all of these costs fall directly to the patient. Hospital costs generally start from $10,000, plus other costs such as the anesthetist which can range from $1,500 &#8211; $2,500. </span></p>

<p><span style="font-weight: 400;">Each provider is different, but typically there is a waiting period for surgery of 12 months. You could then decide whether paying 12 months of Gold tier health insurance would be worth it to you to offset the hospital costs. </span></p>
<p><span style="font-weight: 400;">For example, if a Gold tier policy was $300/month this would be $3,600 for the 12 months. This would save you a minimum of $6,400 in hospital costs if you were willing to wait 12 months for surgery. If it was $500/month you would still be saving a minimum of $4,000 in hospital costs. </span></p>
<p><span style="font-weight: 400;">It&#8217;s important to note at this point, that other options such as obesity treatment using weight loss medication may be available after as little as 6 months with a health insurance provider, which means you could start your weight-loss journey a long time before you can have surgery. It is worth making an appointment with a specialist to discuss your options, then seeking a health insurer that would provide the most benefit during your waiting period for surgery. </span></p>

<p><span style="font-weight: 400;">If you choose not to have private health insurance or do not have the appropriate level of cover, there are still other options available for paying for your surgery. </span></p>
<p><span style="font-weight: 400;">The hospital and your surgeon/clinic will require payment in full ahead of your surgery date (typically one week before). Once you have had your consultations with your bariatric surgeon or specialist, they will be able to provide you with a comprehensive quote for your surgery, including all hospital costs. </span></p>

<p><span style="font-weight: 400;">You can pay your surgery costs in instalments ahead of your surgery date with a payment plan. Many of us are used to booking and paying for a holiday six months or more in advance, so the principle is familiar to us. By paying for your surgery this way you aren’t incurring any debt from credit suppliers, paying additional interest or having to access existing savings (or superannuation). </span></p>

<h3><span style="font-weight: 400;">What is a gastric bypass? </span></h3>
<p><span style="font-weight: 400;">The <a href="https://bariatric.uppergisurgery.com.au/bariatric-surgery/gastric-bypass-surgery/" target="_blank" rel="noopener">gastric bypass</a> (roux-en-y gastric bypass, RYGB) works in two ways to help a patient lose weight. It considerably reduces the size of the stomach, which reduces the volume of food that can be consumed. As the operation essentially ‘bypasses’ the stomach with the early passage of food into the intestine, the hormonal component helps appetite suppression, making weight loss relatively straightforward for the great majority of people. </span></p></div>

<p><span style="font-weight: 400;">The ‘mini’ gastric bypass, also known as the One Anastomosis Gastric Bypass (OAGB), Single Anastomosis Gastric Bypass (SAGB) is similar to a Roux-Bypass but has a somewhat simpler construction. The first part involves creating a narrow gastric tube or pouch, similar to what happens with a roux-bypass, however it&#8217;s often a little longer to help prevent post-op bile reflux. The second part involves joining this “new stomach” to a loop of small bowel, bypassing 150-250 cm. The amount of small bowel bypassed is usually longer than what is bypassed for roux-bypass.</span></p></div>

<p><span style="font-weight: 400;">As a mini gastric bypass is a simpler procedure and can be performed in a shorter amount of time than the traditional roux-en-y bypass, the risks of this bariatric surgery are lower as are potential complications during surgery. A mini bypass has also demonstrated a lower risk of obstruction of the small bowel compared to a standard bypass. </span></p>
<p><span style="font-weight: 400;">The mini gastric bypass also comes with its own potential problems following surgery, such as a risk of suffering from nutrient deficiencies if you don’t keep up with your vitamin and mineral supplements &#8211; like with any bypass this is a requirement for the rest of your life. </span></p>
<p><span style="font-weight: 400;">While a mini gastric bypass has lower risks of abdominal pain and bowel blockages, there is a higher risk of reflux (although less risk than what we see with sleeve gastrectomy) and possibly a bigger risk of stomach ulcers than roux-bypass if patients drink alcohol, smoke cigarettes or take anti-inflammatory medications. The more small bowel that is bypassed during the procedure the greater the chances also of malnutrition, diarrhoea and foul smelling gas.</span></p></div>

<p><span style="font-weight: 400;">The mini gastric bypass has not been performed as long as the roux-en-y gastric bypass, and although it has delivered excellent weight loss results so far, the long-term success can’t be measured as efficiently as the standard operation. </span></p>
<p><span style="font-weight: 400;">Not all <a href="https://bariatric.uppergisurgery.com.au/our-practice/doctors/" target="_blank" rel="noopener">bariatric surgeons</a> are experienced in performing the mini gastric bypass, so you may find it challenging to find a surgeon that has high case load experience in this area, delivering safe and effective obesity treatment. </span></p>

Although many of these procedures are designed to be permanent, in reality not all the effects of the surgery are permanent. All patients perceive their operation to change as months and years pass.</span></p>
<p><span style="font-weight: 400;">These surgeries change how the oesophagus, stomach and small bowel work. Over time these change their function, stretching, softening and adapting to the surgery. Some of these changes can lead to patients developing risks for complications such as ulcers, reflux and nutritional problems, others lead to the operation not seeming quite as “powerful” at controlling how you eat.</span></p></div>

<p><span style="font-weight: 400;">All bariatric surgery procedures suppress appetite and make you eat slowly. If the person learns to eat small, regular meals (three meals a day) and avoid snacking and liquid calories, they will generally do well in the long term. </span></p>
<p><span style="font-weight: 400;">For most patients with weight problems, the tendency to snack, graze and comfort eat was a major contributor to their original weight problem. If these behaviours persist after surgery we expect their weight problem to come back. Some weight regain occurs in the </span><i><span style="font-weight: 400;">large majority</span></i><span style="font-weight: 400;"> of patients after surgery, usually due to snacking rather than eating large meals. </span></p>
<p><span style="font-weight: 400;">Unfortunately it is very common for patients to drift away from their initial enthusiasm and gradually stop looking after themselves and their operation. This weight regain can lead to problems with how the operation works, and complications that require surgical intervention. </span></p></div>

<p><span style="font-weight: 400;">It&#8217;s normal for all of us to spend periods of time when we fail to “self-care” appropriately. All of your care team are human and make similar mistakes, which is why we are happy to try to help when things don’t work as well as you would have liked. </span></p>
<p><span style="font-weight: 400;">Being compliant and sensible following bariatric surgery does not guarantee a hassle-free operation, although it will </span><i><span style="font-weight: 400;">reduce the risk of problems by about 75%</span></i><span style="font-weight: 400;">. Many of the older types of operation (stomach stapling, older banding operations) had design faults that led to problems in their function over time, even if patients did the right thing. </span></p>
<p><span style="font-weight: 400;">In general, between about 10% &#8211; 30% of patients having any type of bariatric surgery will end up needing later treatments to treat complications or weight regain. One of the main reasons patients should return to their bariatric team for follow-up is to reduce the risk of repeat surgery and to notice and treat any problems early.</span></p></div>

<p><i><span style="font-weight: 400;">“Revision surgery is quite common. To date I have performed about 1,000 re-operations on weight loss patients – I usually do two or three of these operations per week.” &#8211; A/Prof Michael Talbot</span></i></p>
<p><span style="font-weight: 400;">Thankfully, many patients seek help when they start to regain weight and can get back on track relatively easily, but there are others who only seek help after they have regained large amounts of weight. </span></p>
<p><span style="font-weight: 400;">This group who only seek help when they have regained significant amounts of weight, probably accounts for about 75% of the patients who need redo surgery after a previous bariatric surgery procedure. They can be easily identified because they have previously stopped coming in for follow-up appointments, they have stopped taking their vitamins and have ceased regular eating and exercise habits.</span></p>
<p><span style="font-weight: 400;">In the other 25% of patients requiring redo surgery, weight regain is due to the development of a technical problem with their operation. If they present before they have had massive weight regain, solving the problem is fairly straightforward.</span></p>

<p><span style="font-weight: 400;">A </span><a href="https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(21)00591-2/fulltext" target="_blank" rel="noopener"><span style="font-weight: 400;">recent article in the Lancet medical journal</span></a><span style="font-weight: 400;"> looked at this question by looking at 174,000 patients with severe obesity who did or didn’t have bariatric surgery. Over 5, 10, 15 and 20+ years those who had bariatric surgery vs those having non-surgical treatments had a 50% less chance of dying. </span></p>
<p><span style="font-weight: 400;">Patients with diabetes who had <a href="https://bariatric.uppergisurgery.com.au/bariatric-surgery/" target="_blank" rel="noopener">bariatric surgery</a> lived almost a decade longer than those who didn’t, which makes sense because yearly “premature deaths” are over 3 x more common in diabetics than non-diabetics, and most people having bariatric surgery will go into remission of their diabetes. </span></p></div>

<p><span style="font-weight: 400;">The rumours are true. Losing weight is an effective treatment for a multitude of medical conditions that obesity can cause;  diabetes, hypertension, high cholesterol, depression,  sleep apnoea and cancer prevention (to name a few). </span></p>
<p><span style="font-weight: 400;">And weight loss surgery is the most effective method for someone living with obesity to lose weight and maintain that weight loss. </span></p>

<p><span style="font-weight: 400;">So although the journey to a healthier, better quality of life isn’t necessarily an easy one, when you lose weight, you will also find that a lot of the medical conditions that you were suffering from before are eased or go into remission completely. For us, that is always completely worth it. </span></p>

If you ask any patients who have had surgery the commonest reply you will hear is “I wish I did it years earlier”. If you ask the weight loss surgeons here at Upper GI Surgery, each of them will tell you the reason they love doing what they do is that they get to witness the change that surgery offers their patients. Being able to see people transform into new, healthier, happier versions of themselves is incredibly rewarding and fulfilling work. </span></p>
